Job Summary:

Responsible for completing client in-home assessments, ensuring that clients meet the qualifications for the different HHOVV Programs, assisting with SNAP applications and submissions, provides additional resources as needed and collaborates with healthcare professionals to coordinate care through the Community Health Initiative.

Duties/Responsibilities:

  • Interviews potential Clients by completing an in-home assessment to determine HHOVV Program, SNAP and/or Community Health Initiative eligibility.
  • Meet with current Clients, annually and as needed for reassessment of all enrolled Program eligibility and well-checks.
  • Meets with qualified Clients and assists with completion and submission of SNAP applications.
  • Maintains accurate SNAP data and submits reports to the HHOVV SNAP Coordinator, monthly and prior to deadline.
  • Collaborates with healthcare professionals and the Community Health Initiative Care Integration Team to ensure proper coordinated care.
  • Documents accurate Client information and submits required documents and reports, monthly to the CHI Senior Manager and/or Finance team for Medicaid billing.
  • Documents accurate referral information within the Apricot platform.
  • Provides referrals to other agencies for additional assistance, as needed.
  • Routinely follows up with Clients, to track progress and ensure satisfaction with HHOVV Programs.
  • Maintains accurate Client database profiles, reviews and updates case notes, documents any challenges & progress notes, Client complaints and APS reporting, as needed.
  • Facilitates and maintains multiple Client care aspects such as drafts and monitors case coordination and service plans, issues and follows-up with outside referrals, and shares information with Program Managers and Intake Coordinators.
  • Develops effective working relations with HHOVV staff, volunteers and other Stakeholders.
  • Submits monthly statistical reports to Program Managers for Grant reporting.
  • Works within a rotation with other HHOVV Case Managers to conduct the UNLV School of Nursing, Community Clinicals class.
  • Works within a rotation with other HHOVV Case Managers within The Marketplace Powered by Helping Hands of Vegas Valley, to provide referral services and additional services to Seniors.
  • Prepares reports by collecting, analyzing, and summarizing statistical data and trends and submits to Director of Monitoring and Evaluation and/or Director of Programs as needed for Grant documentation.
  • Is proactive in enhancing oneself and HHOVV’s reputation, by seeking additional job knowledge, participating in educational opportunities, reading professional publications, and participating in professional organizations.
  • Professionally represents HHOVV and participates in outside vendor events, Senior and Community Fairs and assists with set up and breakdown of all HHOVV equipment.
  • Is comfortable as a mandated reporter to Adult Protective Services and/or local law enforcement agencies, if abuse is suspected.
  • Performs additional duties and responsibilities as necessary to achieve departmental and organizational goals.
  • Develop individualized care plans that incorporate evidence-based practices such as motivational interviewing and crisis management strategies.
  • Provide crisis intervention to de-escalate urgent situations and connect clients with appropriate emergency or ongoing support services.
  • Coordinate with healthcare providers, social service agencies, and community organizations to facilitate access to resources like Section 8 housing, Medicare, workforce development programs, and childcare assistance.
  • Offer ongoing mentoring, behavioral health counseling, and addiction counseling to promote recovery and resilience.
  • Monitor client progress regularly, adjusting care plans as needed while maintaining detailed documentation of interactions and outcomes.
  • Supervise junior staff or interns when applicable, ensuring quality service delivery and adherence to organizational policies.

Required Skills/Abilities:

  • Knowledge and experience dealing with frail, elderly, disabled, and chronically ill clients.
  • Knowledge of modern office procedures and methods including telephone communications, office systems, and record keeping.
  • Experience in using a personal computer and various software packages, including Microsoft Office and database software.
  • Ability to establish priorities, work independently, and proceed with objectives without supervision. Strong attention to detail and follow-through skills.
  • Ability to handle and resolve recurring problems. Ability to contribute to building a positive team spirit.
  • Requires written, verbal, and organizational skills, and the ability to communicate effectively verbally and in writing.
  • Highly organized with the ability to implement systems and follow-up processes, able to effectively work under pressure, use independent judgment and produce a quality work product within tight time constraints.
  • Ability to maintain records and provide clear and concise reports.
  • Must be able to maintain a high degree of confidentiality.
  • Must have highly developed skills in interpersonal relations, writing, and in dealing with clients.
  • Must be able to work well with peers, clients, volunteers, and referral sources. Ability to work effectively in a team environment.
  • Must have reliable transportation and be willing to travel within 100 miles of the Las Vegas Valley, up to 50% of the time, to Clients homes, Vendor/Senior Fairs, Conferences and other events deemed necessary by HHOVV. Mileage is paid at the current IRS Mileage Rate.

Education and Experience:

  • BS degree in Social Services (Social Work, Human Services, Psychology, or other Health Science degree) preferred or a minimum 3 years of case management/intake experience preferred.
  • Current Nevada Community Health Worker Certification is preferred or willing to obtain Nevada CHW Certification within 3 months of hire.
  • High School Diploma or Equivalent required.
  • Knowledge of community resources in Southern Nevada for older Nevadans preferred.
  • Bilingual (English/Spanish/Tagalog) required.
  • Prior experience with Medicaid billing preferred.

Physical Demands:

  • Ability to stand, walk, sit, and use hands to finger, handle, or feel objects, tools, or controls.
  • Ability to manually operate and use a phone, computer, and other office equipment.
  • Ability to talk, clearly hear, and understand telephone conversations.
  • Ability to stoop, kneel, crouch, crawl or lift/move items weighing up to 50 lbs.
  • Team members must be able to work varied schedules to reflect the needs of the organization, to include possible nights and weekends.
  • The noise level in the work environment is usually moderate.
  • Case Managers will work from different environments to include office, home, The Marketplace Powered by HHOVV, Vendor locations, etc.

Work Authorization/Security Clearance Requirements:

  • Must be authorized to work in the United States and must be able to provide 2 forms of ID.
  • Must maintain a valid Nevada Driver’s License and current auto insurance.
  • All positions that drive for HHOVV must authorize HHOVV to run a DMV Motor Vehicle Report every 6 months or as needed
  • Must be able to pass a Federal and State background check.
  • Must be able to pass a hair and urine drug test to include marijuana and THC products. Must be willing to submit to random drug testing if requested.
  • Must be willing to obtain CPR/First Aid/Blood borne Pathogens certification (if not current, must be able to obtain in the first 6 months of employment).

Pay: $45,000.00 - $50,000.00 per year
