Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
Community Health Navigators (CHNs) are frontline public health workers, typically sharing roots with the community in which they serve as a liaison/link/intermediary between health/social services and the community by providing a range of activities such as outreach, community education, informal counseling, social support and advocacy to facilitate access to services that build individual and community capacity and improve the quality and cultural competence of service delivery.
REQUIREMENTS:
- Bachelor’s degree preferred
- One year of experience working as a Community Health Worker, Peer Recovery Coach or Specialist, Care Coordinator, Health Navigator, Community Based Prevention Specialist, Outreach Worker, or Case Manager.
- Personal or family lived experience with navigating the healthcare system due to a physical and/or behavioral health condition is preferred and willingness to share such with others.
- Knowledge of the resources, support, and services available in Wayne County, preferred.
RESPONSIBILITIES (Summary of):
COMMUNITY
- Attain competence, certification where required, in core community training curriculum, including but not limited to Question, Persuade and Refer (QPR), Adult and Adolescent Mental Health First Aid, Whole Health Management (WHAM) and Zero Suicide and provide training as assigned per the training calendar.
- Deliver assigned trainings in the community as scheduled.
- Serve as the liaison between Hegira Health and communities, e.g. social services departments, coalitions, educational institutions, health care organizations.
- Continuously expand knowledge and understanding of community resources, services and programs to contribute to the ongoing development of an agency-wide coordinated approach to community services.
INDIVIDUAL
- Conduct comprehensive screening/assessment with each person served.
- Provide outreach education, identification of needs, and promotion of health and wellness.
- Assist in the coordination of integrated care by meeting with consumers and their care team.
- Help individuals and families, develop their capacity and access to resources, including health insurance, food, housing, quality care, health information, etc.
- Assist assigned consumers with adherence to health care recommendations and with meeting health-related goals.
- Facilitate communication and consumer empowerment in interactions with healthcare/social service systems and the like.
- Advocate for the needs of the consumers.
- Facilitate consumer WHAM groups.

Pay: $40,000.00 - $48,000.00 per year
