General Job Description:


The Resource Navigator provides direct support to individuals and families experiencing homelessness or housing instability by helping them access housing, benefits, and other critical community resources. This role conducts assessments, coordinates services with community partners, and maintains accurate documentation in alignment with Community & Home Supports (CHS) standards and funder requirements.


 


Qualifications:



  • Some college coursework in social work, human services, psychology, or a related field is preferred but not required.

  • Candidates who are certified with the State of Michigan as a Peer Support Specialist; have lived experience of homelessness; and/or have experience working directly with individuals and families who are homeless, diagnosed with a mental illness, battling substance use disorders, or experiencing trauma-related issues are strongly encouraged to apply.

  • Experience in case management, outreach, or navigation within homeless services, behavioral health, or human services settings is preferred but not required.

  • Strong interpersonal skills with the ability to build rapport, establish trust, and maintain professional boundaries with diverse populations.

  • Strong written and verbal communication skills, including accurate documentation, note-taking, and professional interaction with clients and partners. 

  • Ability to interpret and apply complex government regulations and program guidelines, and to explain them clearly to clients and community partners.

  • Proficiency with computer software, including Microsoft Office (Word, Excel, Outlook), and the ability to learn specialized systems such as HMIS and other data platforms.

  • Strong organizational and time-management skills with the ability to manage multiple cases, deadlines, and documentation requirements.

  • Ability to work independently in the field and collaboratively within a multidisciplinary team.

  • Commitment to trauma-informed care, harm reduction, cultural humility, and the mission of CHS to support individuals experiencing homelessness and poverty.


 


Duties and Responsibilities



  • Attain and maintain a comprehensive understanding of federal, state and local regulations related to the definition of “homeless,” “chronically homeless,” and “at risk,” and apply them correctly across homeless services programs.     

  • Develop, maintain, and strengthen collaborative relationships with community partners and external organizations.

  • Coordinating services with community partners to ensure clients have access to housing, healthcare, behavioral health, income supports, and other needed resources.  

  • Meet with individuals experiencing homelessness at their primary nighttime residence (e.g., shelters, encampments, places not meant for habitation) to complete assessments and provide navigation services. Maintain complete, timely, and accurate case documentation for each client file in accordance with agency, funder, and regulatory requirements.    

  • Enter and update client information in the Homeless Management Information System (HMIS) and other required data systems for all assigned cases.

  • Ensuring all assigned client files are organized, current, and audit-ready.    

  • Transport and/or accompany clients to access community resources and appointments as necessary.

  • Participate in required initial and ongoing training in areas such as Housing First, trauma-informed care, harm reduction, and cultural humility.

  • Review and evaluate cases with the Lead Resource Navigator, seeking guidance and collaboration as needed.

  • Attending relevant community meetings, expositions, and conferences as needed.  

  • Perform other related tasks as assigned.

  • Moderate noise (i.e., business office with computers, phone, and printers, light traffic). 

  • Ability to work in a confined area. 

  • Ability to sit at a computer terminal for an extended period. 


 


Physical requirements: 



  • While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard, reach, stoop, kneel to install computer equipment. 

  • Specific vision abilities required by this job include close vision requirements due to computer work. 

  • Light to moderate lifting in required.
