Job Summary

Heavenly Hands is seeking a proactive, compassionate, and detail-oriented Service Coordinator to join our growing team. The Service Coordinator plays a critical role in ensuring clients receive high-quality services by coordinating care, maintaining accurate records, communicating with providers and families, and supporting compliance with agency and regulatory requirements.

The ideal candidate is organized, customer-focused, and able to manage multiple responsibilities in a fast-paced environment while maintaining professionalism and confidentiality.

Responsibilities

  • Coordinate services for assigned clients and ensure service plans are implemented effectively.
  • Maintain regular communication with clients, families, caregivers, and community partners.
  • Schedule and coordinate appointments, assessments, and service visits.
  • Monitor service delivery and address concerns in a timely manner.
  • Maintain accurate and up-to-date client records and documentation.
  • Enter and manage data within agency systems and databases.
  • Assist with client intake, assessments, and service plan updates.
  • Ensure compliance with agency policies, Medicaid requirements, and applicable regulations.
  • Collaborate with interdisciplinary team members to support client goals and outcomes.
  • Prepare reports and track service-related metrics as needed.
  • Participate in staff meetings, training sessions, and quality improvement initiatives.

Qualifications

  • Associate's required , Bachelor's degree preferred in Psychology, Human Services, Social Work, Healthcare Administration, or a related field preferred.
  • Previous experience in service coordination, case management, healthcare, home care, or human services preferred.
  • Strong organizational and time-management skills.
  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft Office Suite and electronic record systems.
  • Ability to manage multiple priorities while maintaining attention to detail.
  • Professional phone etiquette and customer service skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation preferred.
