Job Summary
Heavenly Hands is seeking a proactive, compassionate, and detail-oriented Service Coordinator to join our growing team. The Service Coordinator plays a critical role in ensuring clients receive high-quality services by coordinating care, maintaining accurate records, communicating with providers and families, and supporting compliance with agency and regulatory requirements.
The ideal candidate is organized, customer-focused, and able to manage multiple responsibilities in a fast-paced environment while maintaining professionalism and confidentiality.
Responsibilities
- Coordinate services for assigned clients and ensure service plans are implemented effectively.
- Maintain regular communication with clients, families, caregivers, and community partners.
- Schedule and coordinate appointments, assessments, and service visits.
- Monitor service delivery and address concerns in a timely manner.
- Maintain accurate and up-to-date client records and documentation.
- Enter and manage data within agency systems and databases.
- Assist with client intake, assessments, and service plan updates.
- Ensure compliance with agency policies, Medicaid requirements, and applicable regulations.
- Collaborate with interdisciplinary team members to support client goals and outcomes.
- Prepare reports and track service-related metrics as needed.
- Participate in staff meetings, training sessions, and quality improvement initiatives.
Qualifications
- Associate's required , Bachelor's degree preferred in Psychology, Human Services, Social Work, Healthcare Administration, or a related field preferred.
- Previous experience in service coordination, case management, healthcare, home care, or human services preferred.
- Strong organizational and time-management skills.
- Excellent verbal and written communication skills.
- Proficiency in Microsoft Office Suite and electronic record systems.
- Ability to manage multiple priorities while maintaining attention to detail.
- Professional phone etiquette and customer service skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation preferred.
