Caseworker Role Overview

As a key member of our interdisciplinary team, you'll help ensure patients and families receive holistic care including emotional, social, and spiritual support.

• Serve as the psychosocial support expert on the interdisciplinary care team
• Provide assessments and care planning based on the emotional, psychological, environmental, and cultural needs of patients and families
• Provide counseling crisis intervention practical resource support aligned with each patient's individualized plan of care
• Work closely with patients physicians nurses chaplains other care team members to support
emotional social well-being patient-family unit Educate families hospice services advance care planning grief coping strategies Assist long-term planning community referrals resource navigation needed Monitor document psychosocial changes patent family situation recommend plan updates appropriate Uphold ethical professional judgment end-life
