Shift Available: Full-Time, Tuesday - Saturday.
Position Description: The Case Manager is responsible for all aspects of our Senior Living community’s wellness programs. In collaboration with the Resident Services Director and the Wellness Administrative Assistant, you will coordinate resident services into a cohesive system that maintains a continuing awareness of each resident’s total wellbeing featuring Hearth Management’s Live More® programming and Think More® programming philosophies.
Qualifications:
- A valid New York LPN or RN license is required.
- Minimum of three (3) years full-time experience in the provision of services to a frail dependent adult population.
- Current certification in CPR, First Aid, and Mantoux preferred.
- Must have a valid driver’s license and access to an automobile during working hours to perform job related duties including, but not limited to perform new admission screens and hospital/rehab screens.
