Salary: $60 - $100 Per Assessment & Treatment PlanDESCRIPTION:The Assessor, under the direction of the Clinical Director, plays a crucial role in evaluating clients' behavioral, mental health, and overall well-being. This position involves conducting comprehensive assessments, developing individualized care plans, and collaborating with a multidisciplinary team to ensure the highest quality of care for our clients.JOB DUTIES:1. Client Assessment:- Conduct thorough and standardized assessments of clients' behavioral and mental healthstatus.- Utilize various assessment tools and techniques to gather comprehensive data on clients'history, symptoms, and functioning.2. Individualized Care Planning:- Develop detailed, individualized care plans based on assessment findings.- Collaborate with clients, families, and other healthcare professionals to establishachievable goals and interventions.3. Documentation and Reporting:- Maintain accurate and up-to-date documentation of all assessments, care plans, andclient progress.- Prepare detailed reports and summaries for use in clinical team meetings and forcompliance purposes.4. Collaboration and Communication:- Work closely with a multidisciplinary team including therapists, case managers, andmedical professionals to coordinate care.- Communicate assessment findings and care plans effectively to clients, families, and teammembers.5. Continued Professional Development:- Stay current with the latest research, trends, and best practices in behavioral and mentalhealth assessment.- Participate in ongoing training and professional development opportunitiesQUALIFICATIONS1. Education:a. Masters degree in Psychology, Social Work, Counseling, or a relatedfield preferred.2. Experience:a. Minimum of 2 years of experience in behavioral health assessment ora related field.3. Licensure:a. Must hold a valid and current professional license (LCSW, LMFT, LPC,or equivalent) in the state of employment.4. Skills:a. Strong clinical assessment and diagnostic skills.b. Excellent written and verbal communication abilities.c. Proficiency in electronic health records (EHR) systems.d. Ability to work independently and as part of a multidisciplinary team.*If your role includes supervision of H2H staff members, in addition to general management support, you are expected to meet once (weekly) with each of your team members for a required supervision discussion.
