What You Must Have:
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State Registered Nurse License
- Two years of experience as an RN in a healthcare/clinical setting, hospice preferred.
- Flexible schedule for late afternoon admits, after hours, weekends and holidays
- Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:
- Review clinical referral information and respond to requests and inquiries as appropriate
- Conduct initial meeting with the patient and family over hospice service and philosophy and obtain consents for care.
- Collaborate with Patient Care Manager, attending physician, and hospice physician regarding the eligibility and appropriateness of client for hospice services
- Develop an initial care plan that establishes immediate goals and interventions, based on nursing diagnosis. Includes the patient and the family in the planning process.
- Initiates communication with attending physicians, hospice staff members and other facilities as needed to coordinate optimal care and use of resources for patient / family members.
- Timely submission of all documentation
- Performance of other duties as required
- Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
