Nephrology Nurse Practitioner (NP)
Cumberland Kidney Specialists is seeking a motivated and compassionate Nurse Practitioner to join our nephrology practice.
This position will work closely with our nephrologist and clinical team to provide care for patients with chronic kidney disease, hypertension, electrolyte disorders, dialysis needs, and other kidney-related conditions. The role may include a combination of outpatient clinic visits, dialysis rounding, and hospital-related care depending on experience and credentialing.
Responsibilities
- Evaluate and manage patients with chronic kidney disease and other renal conditions
- Perform patient assessments, physical examinations, and review medical histories, laboratory results, and medications
- Develop and assist with individualized treatment plans in collaboration with the supervising nephrologist
- Provide follow-up care for established nephrology patients
- Participate in the care and management of hemodialysis and peritoneal dialysis patients
- Round at dialysis facilities as assigned
- Educate patients and families regarding kidney disease, medications, diet, fluid management, and treatment recommendations
- Order and review appropriate laboratory testing and diagnostic studies
- Prescribe medications within scope of practice and applicable state and federal requirements
- Communicate and coordinate care with physicians, nurses, dialysis facilities, hospitals, pharmacies, and other healthcare professionals
- Maintain accurate and timely documentation within the electronic medical record
- Assist with hospital and transitional care follow-up as needed
- Maintain compliance with HIPAA, clinical documentation, and practice standards
Qualifications
- Current Nurse Practitioner license or eligibility for licensure in Tennessee
- Family Nurse Practitioner (FNP), Adult-Gerontology, Acute Care, or similar certification
- Current national board certification
- Current or eligible NPI and Medicare enrollment
- Nephrology or dialysis experience preferred, but not required
- Previous internal medicine, hospital, ICU, or chronic disease management experience is a plus
- Strong clinical assessment and communication skills
- Comfortable managing patients with complex and multiple chronic medical conditions
- Ability to work collaboratively with physicians, nursing staff, dialysis teams, and office staff
- Experience with electronic medical records required; AthenaOne experience is a plus
- Strong organizational and documentation skills
- Dependable, professional, and patient-focused
About the Position
This is an excellent opportunity for an experienced Nurse Practitioner or a motivated provider interested in developing experience in nephrology. Training and physician support will be provided as the provider becomes familiar with our practice, patient population, dialysis facilities, and nephrology workflows.
