Do you have what it takes to be a Care Coordinator:
- Completion of a Medical Assistant education program or other Certification/Registration preferred
- Experience in Healthcare, primary care preferred
A day in the life of a HSO Care Coordinator:
- Provide telephone, electronic and in-person outreach and consultation to patients who have open orders
- Assist in identifying gaps in care, provide barrier resolution
- Collaborate with healthcare team to optimize patient adherence to medical plan of care
- Educate patient and family members about preventative screenings, identify barriers to care
- Interacts with healthcare team
- Great communication skills telephonically and electronically with accurate messaging to patient and healthcare team
