🩺 What You’ll Lead

As the Director of Patient Services, you will oversee all clinical operations, performance improvement initiatives, staff training, and regulatory compliance across the home care program.


Clinical Program Leadership

  • Oversee and enhance clinical programs for the delivery of safe, high-quality home care
  • Ensure comprehensive client assessments and appropriate admissions
  • Develop, implement, and monitor individualized plans of care
  • Coordinate interdisciplinary services and ensure continuity of care
  • Maintain compliance with Federal, State, and accreditation standards
  • Supervise nursing and paraprofessional field staff
  • Provide coverage for staff nurses when needed
  • Ensure documentation meets regulatory requirements


Performance Improvement & Quality Initiatives

  • Lead the development and execution of quality improvement strategies
  • Use data-driven approaches aligned with CMS and NYS DOH standards
  • Oversee complaint, incident, and infection control processes
  • Analyze performance data and implement corrective actions
  • Chair or participate in PI committees, preparing agendas, reports, and follow-ups


Orientation, Education, and Competencies

  • Oversee onboarding for field staff and ensure role clarity and policy adherence
  • Lead competency assessments during orientation, annually, and as needed
  • Develop in-service education programs that meet regulatory and accreditation standards
  • Conduct supervisory visits and evaluate clinical performance


Staff Health & Risk Mitigation

  • Ensure required health assessments and documentation for staff
  • Counsel employees on health or exposure-related concerns
  • Respond appropriately to exposure incidents


Customer Service & Operational Excellence

  • Evaluate referrals and ensure timely service responses
  • Oversee satisfaction surveys and related follow-up
  • Promote efficient use of supplies and cost-effective operations
  • Collaborate on staffing patterns and operational workflow


🎓 What You Bring

  • Registered Nurse (RN) licensed in NYS
  • Bachelor’s degree in Nursing
  • 3 years of clinical supervisory experience
  • 1 year of home care experience required
  • Valid NYS driver’s license and reliable transportation
