Responsibilities
- Deliver physical therapy assistant services per the plan of care under supervision of a licensed Physical Therapist
- Assist with patient evaluations, contribute to treatment plan implementation, and document progress toward goals
- Provide therapeutic exercises, gait training, balance work, ADL training, and modalities as indicated
- Monitor patient response, communicate changes to the PT and care team, and adjust activities within scope
- Maintain accurate records, ensure safety and infection control, and manage equipment and supplies
- Collaborate with nursing staff and other therapists to support function, mobility, and discharge readiness

Requirements
- Active Physical Therapy Assistant license in New Hampshire or eligibility per state requirements
- Associate degree in Physical Therapy Assistance
- Relevant experience in LTAC or long-term care rehabilitation preferred
- Ability to work effectively in a long-term acute care / skilled nursing setting
- Strong communication and teamwork skills
