Responsibilities
- Conduct comprehensive PT evaluations for residents within the assignment, establishing baseline measures of mobility, strength, balance, gait, pain, and functional independence.
- Develop individualized, measurable treatment plans aligned with resident goals and facility protocols; select evidence-based interventions and adjust as progress and safety dictate.
- Implement a broad range of therapeutic modalities including therapeutic exercise, gait and balance training, manual therapy, and neuromuscular re-education; document outcomes in the patient record.
- Monitor and document resident progress at regular intervals; communicate changes to the interdisciplinary team; participate in care conferences and update plans of care per regulatory requirements.
- Collaborate with nursing staff, physicians, social work, and therapy assistants to optimize discharge planning and transition strategies; ensure appropriate referrals and community resources when applicable.
- Ensure compliance with infection control, safety standards, documentation accuracy, patient privacy, and regulatory requirements; manage equipment and maintain a clean, safe therapy space.

Requirements
- Doctor of Physical Therapy (DPT) degree from an APTA-accredited program.
- Active physical therapy license to practice in the United States or eligibility for Wyoming licensure.
- Current CPR/BLS certification.
- Experience with geriatric or long-term care populations is preferred.
- Strong written and verbal communication skills; proficient in standard documentation and billing practices.
- Ability to work independently in a travel assignment and adapt to new teams; flexible for a day shift schedule.
