Responsibilities
- Administer prescribed medications and monitor for adverse reactions per physician orders
- Assess residents, document findings, and update care plans with the interdisciplinary team
- Provide routine nursing care including vital signs, wound care, and wound assessment as appropriate
- Assist residents with activities of daily living and maintain dignity and safety
- Collaborate with RNs, LPNs, CNAs, and physicians to coordinate individualized care
- Document care delivered accurately using electronic health records
Requirements
- Active LPN license to practice in Vermont (or eligible to practice in VT)
- Experience in long-term care or geriatric populations preferred
- Ability to work in a long-term care setting and handle a flexible assignment
- Strong communication and documentation skills
- Proficiency with electronic health records or charting systems
