Responsibilities

- Collaborate with RNs and the interdisciplinary team; report changes and escalate concerns promptly
- Assist residents with activities of daily living while maintaining safety and dignity
- Maintain accurate charting and complete incident or change-of-condition reports as required
Requirements
- Active Licensed Practical Nurse (LPN) license in good standing
- Current Basic Life Support (BLS) certification
- Prior experience in long-term care or a closely related setting preferred
- Ability to work a day shift schedule with every-other-weekend coverage
- Strong communication and documentation skills
- Ability to perform in a fast-paced, patient-centered environment
