Responsibilities
- Deliver direct patient care to residents in a long-term care setting, including routine assessments and monitoring.
- Administer medications and treatments in accordance with physician orders and facility policies.
- Monitor vital signs, observe for changes in condition, and report findings promptly to the supervising nurse.
- Document care activities, patient status, and interventions clearly in the charting system.
- Assist residents with activities of daily living, promote resident safety, and support rehabilitation goals.
- Collaborate with registered nurses, physicians, and other members of the care team to develop and implement individualized care plans.
- Ensure infection prevention, safety, and comfort standards are maintained during night shifts.

Requirements
- Active Licensed Practical Nurse license in Vermont (in good standing)
- Graduate of an approved LPN program
- Prior long-term care or skilled nursing facility experience preferred
- Ability to work weekends and night shifts as required by the assignment
- Strong communication, observation, and documentation skills
- Ability to meet facility compliance requirements (e.g., background check)
