Responsibilities

• Screen, evaluate, re-evaluate, and treat patients; coordinate and communicate treatment activities with the interdisciplinary team and the patient
• Develop a care plan in collaboration with the patient’s physician, the patient, and family or primary caregivers at admission; recertify the plan as appropriate
• Coordinate the patient’s care plan through interdisciplinary communication and case conferences; communicate discharges in a timely manner to other disciplines
• Provide ongoing assessment of clinical, functional, and emotional status to identify problems and progress toward goals; document evaluations, goals, and objectives
• Teach the patient and caregiver self-care techniques; provide medication, diet, and other instructions as ordered by the physician; offer health counseling opportunities to patients and caregivers
• Communicate significant changes to the patient’s physician and VNH colleagues; ensure timely documentation and follow-up actions
  
  

Requirements

• Active RN license in Vermont and New Hampshire, or an unencumbered compact license valid in both states
• Minimum 2 years of Home Health experience
• Graduate of an accredited school of nursing
• Ability to work in a home health/hospice setting and participate in a travel/field-based role
• Strong assessment, communication, and care coordination skills with the interdisciplinary team
