Responsibilities

• Assess residents’ physical therapy needs and develop individualized treatment plans within the SNF setting
• Implement therapeutic interventions to improve mobility, strength, balance, and functional independence
• Provide gait training, therapeutic exercises, manual therapy, and assistive device instruction as indicated
• Reassess progress regularly, adjust plans, and document outcomes in electronic health records
• Collaborate with an interdisciplinary team to optimize care and discharge planning
• Maintain safety, infection control, and patient-care standards; educate patients and families as appropriate
  
  

Requirements

• Doctor of Physical Therapy (DPT) degree or equivalent
• Active Maryland Physical Therapy license in good standing (or eligibility)
• Ability to work effectively in a skilled nursing facility environment
• Experience in geriatric rehabilitation or SNF settings preferred
• Strong communication, assessment, and documentation skills
