Responsibilities

• Perform patient evaluations and develop individualized physical therapy treatment plans for geriatric residents
• Implement therapeutic interventions, monitor progress, and adjust plans based on resident response
• Reassess and document outcomes, maintaining accurate records in the medical chart
• Collaborate with the interdisciplinary care team to coordinate care and support discharge planning
• Educate residents and families on therapy goals, safety, and activity programs; promote function and quality of life

• Active Physical Therapist license in good standing or eligible for licensure in West Virginia
