Responsibilities

• Evaluate patients in the outpatient setting and develop individualized treatment plans for a variety of diagnoses
• Implement therapeutic interventions to improve strength, range of motion, balance, safety, and functional function
• Monitor patient progress, adjust goals, and document outcomes
• Educate patients and caregivers on home exercise programs and safe activities
• Collaborate with the care team to coordinate care and referrals as needed
• Maintain accurate patient records and ensure compliance with standards of care
  
  

Requirements

• Bachelor’s degree in physical therapy
• Current Colorado PT licensure (DORA) or Compact for Colorado
• BLS certification through the American Heart Association
• At least two years of PT experience; 3–5 years preferred
• Ability to work in outpatient rehabilitation within a hospital setting
• Travel assignment eligibility: permanent address at least 75 miles from the facility
• Colorado CAPS registration may be required; apply ASAP (may take up to two weeks)
• Ensure correct SSN and date of birth in the onboarding system (Medefis) to prevent processing delays
