Responsibilities

• Perform comprehensive OT evaluations and develop individualized treatment plans for adults in acute rehab within a SNF setting
• Deliver evidence-based therapy to improve self-care, mobility, daily living activities, and functional independence
• Reassess progress regularly, document outcomes, and adjust treatment plans accordingly
• Collaborate with an interdisciplinary team (PT, nursing, social work) to coordinate patient goals and discharge planning
• Educate patients and families on home exercise programs, safety strategies, and equipment needs
• Ensure timely, accurate documentation and compliance with regulatory and facility policies
  
  

Requirements

• Active Occupational Therapy license in New Hampshire or eligibility for licensure
• Degree in Occupational Therapy from an ACOTE-accredited program
• 1 year of related experience in acute care or SNF settings is preferred
• Ability to work effectively in a skilled nursing facility environment and collaborate with a multidisciplinary team
• Proficiency with standard rehab documentation and outcome measures
