Responsibilities

• Assess patients’ functional abilities and develop individualized treatment plans to address ADLs, mobility, and cognition
• Implement therapy interventions to improve independence and safety, with progress tracking and plan adjustments as needed
• Collaborate with an interdisciplinary team to coordinate care, set goals, and support discharge planning
• Document sessions, progress notes, and outcomes in accordance with regulatory and facility standards
• Educate patients and families on home exercise programs, safety, transfers, and equipment use
  
  

Requirements

• Active Occupational Therapy license in Maryland or eligibility to obtain Maryland licensure
• Ability to work in a skilled nursing/acute care setting and participate in a team-oriented environment
• Strong communication, documentation, and time-management skills
• Availability for the 08:00–16:30 shift and travel assignment requirements
