Responsibilities

• Deliver direct nursing care to residents in an LTC setting under the plan of care and supervision of a registered nurse
• Administer prescribed medications and monitor patient responses; document in the chart accurately
• Monitor vital signs, detect changes in condition, and escalate to the supervising RN as needed
• Assist with activities of daily living and perform routine nursing procedures per scope of practice
• Collaborate with the interdisciplinary team (nursing, therapy, dietary, social services) to support resident well-being
• Maintain infection control, safety standards, and resident rights; ensure accurate and complete care documentation
  
  

Requirements

• Active LPN license in New Hampshire in good standing
• Graduation from an accredited practical nursing program
• Prior clinical experience as an LPN; long-term care experience preferred
• Ability to work evenings (3:00 PM–11:30 PM) and adapt to the assignment schedule
• Willingness to travel to and work a travel assignment in Rochester, NH
