Responsibilities

• Provide direct patient care to long-term care residents under the supervision of an RN or physician, including assessment, observation, and response to evolving conditions
• Monitor vital signs, intake and output, neurovascular checks, and report changes promptly using the facility’s charting system
• Administer medications and treatments as prescribed, document administration accurately, verify MARs, and participate in medication reconciliation as needed
• Assist with activities of daily living (ADLs) such as bathing, grooming, dressing, mobility assistance, and feeding, while promoting dignity and independence
• Maintain accurate nursing notes, care plans, and progress notes; update documentation to reflect changes in condition and interventions
• Collaborate with the interprofessional team (CNAs, RNs, physicians, therapy staff) to coordinate care, plan discharges, and ensure patient safety and quality outcomes
  
  

Requirements

• Active LPN license
• Ability to work the assigned three 12-hour day shifts per week
• Comfort working in a long-term care setting with frail adult residents
• Proficiency with nursing documentation and electronic health records
• Strong communication, teamwork, and critical-thinking skills
