Responsibilities

• Administer prescribed medications and treatments per plan of care; monitor resident responses and document outcomes
• Provide direct patient care including activities of daily living, comfort measures, and mobility assistance
• Monitor residents’ vital signs and overall condition; recognize changes and escalate to the supervising nurse promptly
• Document care activities, update care plans, and ensure accuracy of resident records
• Collaborate with the interdisciplinary team to coordinate care and maintain safety and infection control standards
• Support residents and families with communication, advocacy, and respectful, person-centered care
  
  

Requirements

• Active Licensed Practical Nurse license (state-specific as required)
• Ability to work night shifts (11.5 hours) for the duration of the assignment
• Willingness to commit to a 4-week contract with guaranteed hours
• Experience in a long-term care or skilled nursing setting preferred
• Strong written and verbal communication skills; ability to document clearly and accurately
• Ability to pass required credentialing and background checks for travel assignments
