Responsibilities

• Assist with and supervise the day-to-day nursing activities during your shift, ensuring care aligns with current standards and regulations
• Administer prescribed medications and treatments in accordance with physician orders and facility policy; monitor for adverse reactions
• Perform resident assessments, document findings, and communicate changes to the nursing team to support care planning
• Implement and support rehabilitative and restorative nursing practices as part of residents’ care plans
• Collaborate with the charge nurse and other care staff to maintain a safe, quality care environment; monitor resident safety and respond to changes in condition
• Complete timely and accurate charting and documentation; support continuity of care across shifts
  
  

Requirements

• Active LPN license in the state of Colorado
• Minimum 3 years of experience in a hospital, skilled nursing facility, or comparable setting
• Training in rehabilitative and restorative nursing practices
• IV certification preferred for LPNs; current BLS certification
• CAPS check (Colorado adult protective services) required
• Ability to work in a long-term care memory care environment
• Reliable transportation available due to rural Gunnison location (no public transportation)
