HealthCare Support is actively seeking an Advanced Practice Clinician-Nurse Practitioner or Physician Assistant to fill an opening in Greensboro, NC.
Daily Responsibilities For NP/PA
- Travel to 4 facilities in Guilford County to perform comprehensive patient assessments, addressing both medical and behavioral conditions.
- Manage acute and chronic illnesses proficiently, ensuring accurate and thorough documentation, including ICD 10 conditions.
- Communicate and collaborate with the interdisciplinary care team, conducting advanced illness and care planning conversations, while providing counseling and education to patients and caregivers.
- M-F 8am-5pm
- Active and unrestricted license in North Carolina
- Graduate of an accredited MSN, DNP, or MPA program
- Board certified NP (through ANCC or AANP) or PA
- Current, active DEA licensure/prescriptive authority
- Full COVID-19 vaccination
- 1 year of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end of life discussions
- Proficient computer skills including the ability to document medical information with written and electronic medical records
