Daily Responsibilities For LSW Field Care Manager Behavioral Health
- Assesses and evaluates members’ needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members.
- Shift: M-F 8AM-5PM
- This is a HYBRID position with Heavy Field Work and Remote Work
- Licensed Bachelor's prepared Social Worker or continuous active limited social worker license with intent to obtain full licensure.
- LMSW
- Minimum of 2 years of post-degree clinical experience in behavioral health setting
- Michigan State licensure in field of study
- Ability to travel to region-based facilities and homes for face-to-face assessments
- Experience working with the adult population, disease management.
- Knowledge of community health and social service agencies and additional community resources.
