HealthCare Support is actively seeking a Field CFSP Care Coordinator to fill an opening in Asheville, North Carolina.

Daily Responsibilities For Field CFSP Care Coordinator

• Supporting Elevance’s health plan in North Carolina – Health Blue of NC, specifically working on the Children and Families Specialty Plan (CFSP)
• Will be in the office 2-3 days a week and then in the field/member homes the rest of the time.
• Provide integrated whole-person Care Management under the CFSP Care Management model, including coordination across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet health-related needs.
• M-F, 8-5 EST
  
  

Required Qualifications for Field CFSP Care Coordinator:

• Bachelor’s Degree required (no flexibility with this per their contract with the state)
• STRONGLY prefer Community Health Worker Certification
• 1 years of experience working with children and families in a community-based role (looking for childcare and/or child welfare experience

Pay Details: $28- $30/ Hour
