HealthCare Support is actively seeking a RN Field Case Manager to fill an opening in Seattle, Washington.
Daily Responsibilities for RN Field Case Manager
- Ensure accurate and complete documentation.
- Communicate and collaborate with the interdisciplinary care team
- Assist the provider/team with various care coordination activities
- Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
- Monday through Friday 8AM - 5PM
- Active and unrestricted RN license in Washington
- Case management background preferred- experience working in a Long-Term Care facility as an RN preferred
- Associate degree or higher in Nursing (RN)
- Certified in Basic Life Support
- 1 year of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Proficient computer skills including the ability to document medical information with written and electronic medical records
- Effective Communication skills and time management

Salary : $55 - $60
