Pediatric RN Case Manager
HealthCare Support is actively seeking a Pediatric RN Case Manager to fill an opening with a Fortune 500 Company in Lubbock, Texas.
Daily Responsibilities for Pediatric RN Case Manager:
- Managing a complex member caseload, conducting clinical and service coordination reviews
- Completion of contractual telephonic and face to face assessments
- Ensure members services and DME items are in place
- Meet audit score goal of 95%
- Supporting issue resolution, provider coordination, member and caregiver education, and proactive identification of care gaps.
- Project involvement may include quality initiatives, audit preparation activities, process improvement efforts, and pilot implementations tied to state or organizational priorities
- Field visits are required
- 2 years of recent pediatric experience, preferably in home health or case management
- 4 years of RN experience (need to convert FTE)
- Good job tenure
- Advanced documentation/technical system proficiency
- Microsoft Office experience
- Clear and active TX RN license
- Clear and active drivers license and reliable transportation and insurance
- Ability to assess needs appropriately
- Accurate Documentation of those needs
Preferred but not a must:
- Pediatric home health
- Case management experience
- Experience with Special Needs Children

Pay Details: $38/hour - $44/hour
