Field Nurse Practitioner / Physician Assistant

You'll serve as the primary care provider for Medicare and Medicaid patients, helping improve health outcomes through in-home visits, care coordination, and chronic disease management.
Openings in Southeast Phoenix and East Valley, AZ (Tempe, Southeast Scottsdale, and Central East Phoenix preferred)
Pay Details: $120,000 - $142,000 annually (DOE)
Eligible for up to 7.5% Annual Bonus
Mileage Reimbursement (IRS rate for travel over 30 miles)
Shift: Monday – Friday | 8am-5pm
No weekends

Daily Responsibilities for the Field Nurse Practitioner / Physician Assistant:
- Complete 5-7 in-home patient visits per day
- Perform history and physical assessments
- Develop and manage treatment plans
- Prescribe medications and order diagnostics
- Provide patient education and chronic disease management
- Coordinate care with the interdisciplinary team
- Complete documentation in Athena EMR
- Conduct occasional telehealth visits as needed
Required Qualifications for the Field Nurse Practitioner / Physician Assistant:
- Active Arizona NP or PA license
- 1 year of primary care, family practice, or outpatient experience
- Active DEA license
- Current BLS certification
- EMR experience
- Reliable transportation and valid auto insurance
- Valid driver's license with acceptable driving record
