As a Social Service Director , you will be responsible for overseeing the Social Services department, which provides emotional support, counseling, and linkage to community resources for our patients. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient. Your expertise will be instrumental in ensuring our patients receive comprehensive care that addresses their physical, emotional, and social well-being.
Responsibilities
Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team
Provide emotional support and counseling to patients and families, addressing emotional, social, and financial concerns
Connect patients with community resources, such as home healthcare, adult day care, and long-term care facilities
Coordinate discharge planning and ensure a smooth transition for patients returning home
Supervise and mentor Social Workers and other support staff to ensure high-quality care and support
Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Maintain accurate and up-to-date records of patient care and services provided
