Supervisor, Mobile Crisis Response Team (LCSW), Street Medicine

At Healthcare in Action, we serve patients experiencing homelessness through an innovative "street medicine" approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.

The Garden Grove Mobile Crisis Response Team (MCRT) is a co-response program operated by Healthcare in Action (HIA) in partnership with the City of Garden Grove and the Garden Grove Police Department (GGPD), delivering mobile mental health and crisis stabilization services in the field.

The program provides eight hours of coverage per day, seven days per week, through two field teams, each pairing an EMT with an associate-level behavioral health clinician-an Associate Clinical Social Worker (ACSW), Associate Marriage and Family Therapist (AMFT), or Associate Professional Clinical Counselor (APCC)-who has prior crisis or field-based behavioral health experience.

The Supervisor provides clinical supervision and day-to-day program management for the Garden Grove MCRT. The Supervisor hires, leads, and clinically supervises both field teams; serves as the primary liaison to GGPD; and acts as the clinical point of escalation for complex or high-risk situations. The Supervisor also provides direct field coverage during planned and unplanned staff absences to maintain uninterrupted service and is present in the field as needed to carry out supervisory, quality, and safety responsibilities.

Responsibilities:

• Provide clinical supervision and leadership to the MCRT field teams, including guidance on triage, assessment, de-escalation, suicide risk assessment, crisis counseling, safety planning, referral, and disposition; provide clinical supervision toward licensure for the associate-level clinicians.
• Hire, onboard, train, and manage the performance of the associate-level clinicians and EMTs.
• Manage day-to-day program operations for both field teams, including staffing schedules, shift assignments, and midweek overlap activities such as joint training, case conferencing, and supervision.
• Provide direct field coverage during planned and unplanned staff absences to minimize disruption to seven-day coverage; the Supervisor is not routinely rostered to a field team but rides along as needed to perform supervisory, quality, and safety duties.
• Serve as the primary liaison to GGPD and City partners, co-developing joint policies, protocols, and training, and maintaining continuous communication with GGPD regarding team location, status, and plan.
• Ensure the team operates safely and within the approved scope of work by responding only after GGPD clears the scene, observing program boundaries (no involuntary holds, no transport of individuals in program vehicles), and reinforcing personal-safety and de-escalation standards.
• Serve as the clinical point of escalation for complex, high-risk, or ambiguous situations, determining when an individual requires a higher level of care.
• Direct appropriate care pathways and dispositions, including referral to a hospital, crisis stabilization unit, community provider, street medicine program, the CAT team, or the Department of Mental Health (DMH).
• Monitor documentation and reporting to ensure each response is documented timely and accurately, program performance is reported to the City and GGPD, and quality standards are maintained for assessments, dispositions, and follow-up.
• Protect confidential information by following all HIPAA regulations; actively support the achievement of SCAN's and HIA's Vision and Goals; and perform other duties as assigned.
• This position is a patient-facing role and a healthcare worker.
• This position will have supervisory responsibilities for 2 workers.
