Shared Living Provider (Live-In) – Windsor County, VT
Health Care and Rehabilitation Services (HCRS), a trusted nonprofit serving Southern Vermont, is seeking a compassionate Shared Living Provider (SLP) to provide a safe, supportive, and inclusive home environment for a young adult with developmental and medical support needs.
This is a live-in opportunity for someone who values relationship-based care and wants to make a meaningful, long-term impact while maintaining flexibility and autonomy.
What You’ll Do:
- Support daily living activities at home and in the community
- Encourage independence and skill-building
- Assist with communication and daily routines
- Provide medication reminders and meal preparation
- Support transportation to school, appointments, and activities
- Collaborate with a professional care team
