Role Description

HealthBridge Personalized Medicine is seeking a full-time, on-site Physician specializing in Concierge Internal or Family Medicine. Based in Great Neck, NY, the physician will provide personalized patient care while working collaboratively with a multidisciplinary team of healthcare professionals. Responsibilities include conducting patient consultations, developing tailored treatment plans, managing chronic conditions, and focusing on preventative care. The role emphasizes maintaining strong patient relationships and delivering exceptional healthcare experiences.

Qualifications

• Board certified in Internal Medicine or Family Medicine or IM/peds
• Proficiency in building and maintaining long-term patient relationships with a personalized care approach
• Skill in diagnosing and managing chronic conditions and developing customized treatment plans
• Strong communication and collaboration skills for working in a multidisciplinary healthcare team
• Commitment to staying informed on advancements in preventative healthcare practices
• Experience in concierge medicine or similar personalized care models is a plus
• Active medical license in the state of New York
