Position Overview
The Patient Liaison plays a central role in delivering the high-touch, personalized experience our members expect. This individual provides daily clinical and administrative support to designated physician partners and their patient panels, serving as a trusted, competent, and warm presence throughout every member interaction.
Job Duties & Responsibilities
Clinical Support
- Patient rooming and clinical assistance during visits
- Phlebotomy, IV placement, and laboratory support
- Administration of point-of-care testing (INR, glucose, ECG, etc.)
- Administration of injections and immunizations
- Physician-assisted prescription management
Administrative & Care Coordination
- Appointment and referral scheduling
- Insurance pre-certifications and prior authorizations
- Verification of current patient insurance information
- Medical records management
- Vendor management
- Open service of additional patient needs as directed by physician and operations leadership
Education, Experience & Skills
- High school diploma required; college degree preferred
- RN, LPN, or Medical Assistant Certification required
- Minimum 3 years of clinical experience required; 5 years preferred
- Proficiency with EMR systems (ideally Elation Health) and Microsoft Office Suite (Excel, Word, Outlook) required
- Must be genuinely service-oriented with a warm, professional demeanor across all member interactions
- Excellent communicator, written and verbal, with exceptional attention to detail
- Self-motivated, proactive problem-solver with the ability to multitask and prioritize in a fast-paced environment
