We are seeking a dedicated Physical Therapist to provide outpatient rehabilitation services primarily in in-home settings for older adults. The role involves evaluating, developing, and implementing personalized therapy programs to promote independence, including therapeutic exercises, manual therapy, and functional training. The therapist will educate patients and caregivers, make referrals to other disciplines, and adapt plans based on individual needs. Supervision of support personnel may be required in accordance with licensing standards.

- Performing reexaminations, designing adaptive devices, and conducting mobility assessments.
- Valid Physical Therapy license in applicable state(s) or eligibility.
- Master’s degree in Physical Therapy or related field, or equivalent experience.
- Reliable transportation and a valid driver’s license.
