Part-Time Home Health Aide (Days)

Role: Home Health Physical Therapist responsible for delivering direct patient care, collaborating with team members, and developing comprehensive care plans.

• Key duties: Provide therapy services using rehabilitation equipment such as splints, orthoses, prostheses, wheelchairs, ADL equipment, and modalities.
• Qualifications: Must have 2 years of acute hospital and home care experience, proficiency with OASIS, valid Driver License, and own vehicle.
• Certifications and Licenses: Current Massachusetts PT license, active BLS (AHA), and current skills checklist.
• Profile requirements: Include work history, recent supervisory reference, Massachusetts license, and BLS documentation.
• Additional info: Must be able to work independently, with weekend and holiday rotation if needed. No call required.
