- Conduct evaluations, develop individualized care plans, and administer evidence-based therapy to enhance mobility, independence, and wellness.
- Document patient progress accurately within electronic medical records, ensuring compliance with regulatory standards.
- Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Utilize telehealth and virtual care to increase access and flexibility.
- Valid Occupational Therapy license and degree from an accredited program.
- Proficiency with computers and EMR systems.
- Current CPR certification.
- Ability to perform physical tasks such as lifting, repositioning, and assisting with transfers.
