Role: Licensed Practical Nurse (LPN) or Registered Nurse (RN) providing in-home care for pediatric and adult medically fragile patients.

- Utilizing the nursing process to assess, plan, implement, and evaluate patient care.
- Delivering skilled nursing services in accordance with prescribed orders.
- Assessing signs of physiologic and psychosocial changes in patients.
- Collecting, analyzing, and documenting healthcare data and nursing diagnoses.
- Current LPN/LVN or RN license in the practicing state.
- Current TB or Chest X-Ray and BLS certification.
- Preferred one year of professional nursing experience.
- Strong assessment, documentation, and patient care skills.
