LPN Homecare - Full-Time Nights - PRN Weekends

Role Overview: The Licensed Practical Nurse (LPN) provides skilled nursing care to adult patients in their homes, utilizing the nursing process to assess, plan, implement, and evaluate care.

Key Responsibilities

• Assess and monitor patient signs and symptoms, including physiologic and psychosocial changes.
• Administer medications and perform tasks related to G-tube care; future responsibilities may include trach/vent management.
• Document patient data and collaborate with healthcare team members to ensure quality care.
  
  

Qualifications & Skills

• Current LPN/LVN license in the practicing state.
• Current TB or Chest X-Ray and BLS certification.
• Preferably, at least one year of professional nursing experience.
  
  

Key Duties: Provide direct patient care in accordance with prescribed orders, assess patient condition, and ensure accurate documentation.
