- Provide direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions and the plan of care.
- Document patient visits thoroughly in electronic medical records and electronic visit verification systems.
- Participate in care coordination by reporting patient needs and condition changes, attending case conferences, and communicating effectively with patients, families, physicians, and team members.
- Deliver high-quality clinical services within the scope of practice, adhering to infection control procedures and state regulations.
- Work independently in diverse home environments, ensuring respectful and quality care regardless of patient demographics.
- Current, unrestricted LPN licensure in the practice state.
- Valid driver’s license, vehicle insurance, reliable transportation or access to public transit.
- Preferred: CPR certification, ability to work flexible hours, strong communication, organizational, and multitasking skills.
