We are seeking Licensed Practical Nurses (LPN/LVN) to provide community-based home health care in various locations, primarily covering the South Valley, Valencia County, and eastern Albuquerque.
Key responsibilities include delivering nursing care under RN supervision, coordinating with interdisciplinary teams, developing and implementing care plans, monitoring patient health, administering medications, educating patients and families, and maintaining accurate documentation.
Qualifications include a diploma from an accredited vocational/practical nursing program, current licensure (including multi-state licensing if applicable), at least two years of clinical experience, proficiency in nursing skills, reliable transportation, and experience with EMR systems like HCHB is preferred.
Work involves home visits, potential exposure to blood and infectious diseases, physical effort such as lifting, and working in various weather conditions.
