Role Overview: A Licensed Practical Nurse (LPN) on the Transition team provides skilled nursing care to patients in their homes during a provisional period of 1-30 days. The team facilitates quick and efficient patient discharge while recruiting full-time home healthcare nurses. Responsibilities include assessing, planning, implementing, and evaluating patient care, administering prescribed services, and monitoring physiologic and psychosocial changes. The LPN collects and interprets data, documents diagnoses, and ensures quality care in accordance with orders.

- Current LPN license in the practice state
- Competency with enteral feeds, tracheostomies, ventilators, and care for adult and pediatric patients
- Current TB or Chest X-Ray and BLS certification
