- Delivering direct patient care, including personal hygiene, mobility assistance, and basic nursing tasks under supervision.
- Assisting with transfers, repositioning, and prescribed exercises.
- Preparing meals tailored to client preferences and dietary needs.
- Offering companionship and engaging in activities suited to individual interests.
- Performing light housekeeping duties and maintaining accurate records.
- Monitoring and reporting changes in clients' physical or psychosocial conditions.
- Following the care plan strictly, including documenting tasks via EVV app or time sheet.
- Ensuring patient confidentiality, privacy, and safety, and reporting any concerns or abuse.
- Using equipment such as blood pressure cuffs and stethoscopes as prescribed.
- Active certification with DFS.
- High school diploma or GED preferred.
- Effective communication skills, ability to read, write, and follow directions.
- Physical ability to lift approximately 50 lbs, bend, stand, and perform necessary tasks.
- Compassionate attitude toward patient care.
