Role Overview: The Hospice Medical Social Worker is a key member of the interdisciplinary team, providing emotional, social, financial, and resource support to patients and families facing serious illness and end-of-life care.
Key Responsibilities: Conduct social casework, assess financial situations, respond to referrals, participate in team and agency conferences, refer to community resources, interpret social needs to staff, and ensure the use of the 4Ms (What Matters, Medications, Mentation, Mobility) for patient-centered care.
Qualifications & Skills: Hold a Master’s degree in Social Work from an accredited program, maintain a current state license, possess excellent clinical judgment and communication skills, and proficiency in computer use. Availability during agency hours is required.
Preferred Experience: Advanced certification in hospice social work, two years of hospice experience, and familiarity with electronic medical records are advantageous.
