Licensed Practical / Vocational Nurse – LPN/LVN

Role Overview: Provide clinical care to patients in home settings, ensuring outstanding clinical outcomes through compassionate and empathetic nursing practices.

Key Responsibilities

• Deliver direct patient care following physician’s orders
• Utilize electronic documentation via tablets in patients’ homes
• Collaborate with a multidisciplinary team to coordinate care
• Participate in paid training, mentorship, and ongoing learning sessions
• Support patients across day, night, weekend, full-time, part-time, or PRN schedules
  
  

Qualifications & Skills

• Active, unencumbered LPN/LVN license in the practicing state (compact licenses transferred within 90 days)
• Current CPR certification (training assistance available)
• TB skin test within the last 12 months
• Reliable transportation
• Preferred: Six months of hands-on nursing experience
