Role Overview: The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physicians' prescriptions and within legal scope.

Key Responsibilities

• Deliver high-quality clinical services aligned with the care plan and coordinate with the healthcare team.
• Document patient visits comprehensively in electronic records, including visit verification.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, clinicians, and management.
  
  

Qualifications & Skills

• Unrestricted LPN licensure in the practice state.
• CPR certification or ability to obtain within 90 days.
• Ability to work independently in diverse home environments.
• Valid driver’s license, vehicle insurance, or access to reliable transportation.
• Strong communication, organizational, and multitasking skills preferred.
