Role: Nurse Practitioner, Advanced Practice Provider
Responsibilities: Provide effective, equitable primary care to adult Medicare patients, including routine and acute office visits, annual wellness exams, and health assessments. Collaborate with a multidisciplinary team, coordinate care with specialists, and lead population health initiatives. Educate patients, assist with phone triage and outreach, conduct home visits when necessary, and participate in promotional activities. Work closely with the Center Medical Director and operational leadership.
Qualifications: Master's or Doctoral degree in Nursing, national certification in relevant specialty, active NP license, DEA license, and US work authorization. Preferred experience in primary care, understanding of HCC, ICD-10 coding, and health risk assessments. Bilingual skills and a passion for teamwork are advantageous.
